    Does anybody know what a “Return to work release form” from Dr. is?

    My manager sent me a TXT saying that when I get ready to return to work I will need a “Return to work release form”.

    I have been feeling so sick that I haven’t worked since June 8th. I miss it so much but my heart rate is in the high 90s and low 100s @ rest. I feel dizzy too and super tired. I’m afraid and getting sick on my way to work or at work.

    Has anyone had to ask for the same form before?

    It would seem that whatever business you work at has a “Return to Work” release form. Is there a Human Resources/Personnel department at your company? That would probably be where you would find such a form.

    Your dr will have a form letter to sign that HR should accept.

    Yes, that is the way it usually works, as NLbatten says.
    But it can vary. Sometimes you just need a note from the doc.
    Ask your boss, or HR.

    Thank you everyone for your replies. I have asked my manager and it is a form I need to get from the Dr. I will have to talk to him about it.
